Contractors, homeowners, lenders, settlement attorneys, and realtors seeking proof of insurance coverage PLEASE CLICK Request a Certificate of Insurance. Complete all requested information, and hit “SEND”.
The certificate will be issued in the order in which it was requested and through the delivery method of your choice (e-mail, fax number or mailing address).
- Full name(s) of the borrower(s)/purchaser(s)/owns(s)/insured(s).
- A complete address of the property, including the city, state, and zip code.
- Complete name of the lender.
- Lender’s loss payee information/mortgagee clause.
- Lender’s complete address, including city, state, and zip code.
- Lender’s loss payee position (1st Mortgagee, 2nd Mortgagee, etc.)
- The borrower’s loan number.
- The preferred method of delivery of your certificate (e-mail, fax number, or mailing address).
Attention Contractors:
1) You can keep your General Liability Insurance and Workers Compensation cost down by collecting and retaining certificates from your subcontractors for your audits.
2) Contractors that obtain a certificate and requests to be listed as a “certificate holder” also need to be aware of the following: “THE ISSUING INSURER WILL ENDEAVOR TO MAIL ___ DAYS WRITTEN NOTICE TO THE CERTIFICATE HOLDER NAMED…, BUT FAILURE TO DO SO SHALL IMPOSE NO OBLIGATION OR LIABILITY OF ANY KIND UPON THE INSURER, ITS AGENTS OR REPRESENTATIVES.”
It is important to note the words “ENDEAVOR TO” it is not a guarantee. Some insurance companies never try to notify the certificate holder in the event of cancellation. Because of this it is important to check on certificates several times during the year and maintain detailed records or during an audit you may be charged an additional premium due to the early cancellation of the subcontractors insurance.
